Organización de empresas Resumen: This book disentangles the issues in connection with the advancement of Health Technology Assessment (HTA) and its interface with health policy. It highlights the factors that should shape its progress in the near future. Interdisciplinary and critical views from a number of professionals are put together in a prescient order to cast some light and make recommendations as to the next steps HTA should take to be fit for purpose. A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. Under the dynamics of global change, megacities are themselves changing: On the one hand they are prone to increasing socio-economic vulnerability due to pronounced poverty, socio-spatial and political fragmentation, sometimes with extreme forms of segregation, disparities and conflicts. On the other hand megacities offer positive potential for global transformation, e.g. minimisation of space consumption, highly effective use of resources, efficient disaster prevention and health care options – if good strategies were developed. At present in many megacities and urban areas of the developing world and the emerging economies the quality of life is eroding. Most of the megacities have grown to unprecedented size, and the pace of urbanisation has far exceeded the growth of the necessary infrastructure and services. As a result, an increasing number of urban dwellers are left without access to basic amenities like clean drinking water, fresh air and safe food. Additionally, social inequalities lead to subsequent and significant intra-urban health inequalities and unbalanced disease burdens that can trigger conflict and violence between subpopulations. The guiding idea of our book lies in a multi- and interdisciplinary approach to the complex topic of megacities and urban health that can only be adequately understood when different disciplines share their knowledge and methodological tools to work together. At a political level, the EU issued the EU Directive no. 24/2011/CE of 9th March 2011 concerning the application of patients’ rights in cross-border health care and has contributed to improving the level of freedom of choice for the European citizen, but it does not seem to have increased actual patient mobility across Europe. Freedom to choose is necessary to grant the people of Europe the same access to public-sector health-care services. The latter is a key instrument for an efficiently functioning “single market” ensuring real mobility within the EU. The aim of this book is to study the current European health care market and discuss the hypothesis of a European right of citizenship with reference to health-care services. It examines patients' mobility from several perspectives: determinants of patient mobility, governance of cross-border mobility at EU level as concerns patients and health-care professionals, policy implications, and case studies. It is intended for health researchers, decision-makers and professionals concerned with health-care provision and patient mobility.
